Dr. Watkins' Approach
Dr. Watkins performs hairline lowering using a pre-trichial technique, in which the incision is placed just behind the fine vellus hairs at the leading edge of the hairline. This allows the scar to be camouflaged by the surrounding hair, and the incision is beveled to encourage hair to grow through it as it heals, further softening its visibility over time. To allow the scalp to advance forward, Dr. Watkins releases the underlying galea (galeotomies), which provides additional mobility and reduces tension on the final closure.
On average, hairline lowering can achieve approximately 2 cm of advancement, though the exact amount depends on each patient's individual scalp laxity and anatomy. The ideal candidate has a congenitally high hairline without significant or progressive hair loss; patients with thinning or unstable hair density may be better suited to hair transplantation or a combination approach, as discussed above.
